Social services are provided by a social worker consultant that is available to help with adjustment, problem resolution and discharge planning. Activities are planned by our Social Activities Director and including spiritual/religious programs, physically stimulating programs, adaptive programming for persons having cognitive or physical disabilities, educational activities, creative/intellectual activities, holiday activities, weekend activities, special events, outings and a volunteer program. Our in-house environmental services include housekeeping, laundry and maintenance. Rehabilitation services include gait training, balance and posture training, pain management and exercise programs. Occupational Therapy provides training in the activities of daily living, use of adaptive equipment, fine eye coordination and hand function.

Assessing hospitals’ clinical risk management: Development of a monitoring instrument
The service, offered by systems subsidiary Toshiba Solutions, will employ a tablet device along with a scale and a blood pressure monitor, both compatible with Bluetooth. Weight and blood pressure readings will be sent to Toshiba Solutions servers. The tablets will display more than six months of data, with an alarm reminding the senior to take medication. Family members will have access to the information as well. Service fees are expected to start at JPY 1,000 per month. Toshiba may later partner with outside firms, such as a food delivery provider for seniors. It aims to sign up 300,000 users within three years for the health management service.

Toshiba to launch tablet health-monitoring service in Japan
Most importantly single sector M&E approaches are often driven by institutions external to the national governments, such as by donors, and those external institutional priorities might not reflect those of each country [ 48 ]. This papers approach requires that program planning and M&E systems be determined based on national level priorities. While this may increase the likelihood that the programmatic response is tailored, it does not mean that donor funding will correspond with the countrys priorities. Further, there may be human and financial resource constraints that limit application of the approach. Because the perspective of this approach is at the country level, it is consistent with calls for greater country ownership [ 1 , 4 ].

The interviewees (five persons responsible for CRM in major Swiss hospitals and the CEO of a risk management company and president of the Swiss risk-management network) were selected according to suggestions from our advisory board. In addition, questionnaire design experts reviewed the draft version regarding wording, structure and order of questions and response alternatives. This was followed by written feedback and a workshop with our expert panel. Their input resulted in further precision in wording and more questions on specific activities with examples instead of abstract concepts or attitudes. An intense discussion took place of how to gain the overview of CRM in different services (section 3 of the instrument).
